Eligibility Criteria:
- Primary life-limiting illness
- 12 month life expectancy or less Homebound per Medicare definition
- Deteriorating medical condition at risk for needing symptom management
- Recent emergency department or hospital stay, or at risk for such
- Consent to emphasis on care in the home setting
Exclusion Criteria:
- Receiving a hospice benefit Nursing home resident
requires more care than can be provided in the home
REFERRAL PROCESS
How to Refer:
- Call 303.604.5272 to refer a patient or to request an optional visit from
a HospiceCare palliative care physician to determine patient appropriateness
Purpose of Service:
- To address symptoms related to physical, emotional, spiritual concerns
- To decrease need for emergency department visits or hospitalization
- To explore and document goals of care
- To educate patient and family re: disease process, preventative care,
medication management, and crisis management in home
- To anticipate changes in condition and have appropriate plan in place
- To help seriously ill patients transition to hospice program of choice if appropriate

Site of Service:
Who Delivers Care:
- Interdisciplinary Team (physician, nurse, nursing assistant, social worker, chaplain) provides home visits for pain and symptom management and helps with planning or the end of life
- Team collaborates with primary physician
Ideal referral:
- Patient who is strongly symptomatic related to primary illness
- Patient who is homebound per Medicare definition
- Recent emergency department visit or hospitalization
- Care provider available
- Increasing burden vs. benefit of treatments
- Until stabilization, admission to hospice
Length of Service:
- Until stabilization, admission to hospice
